2009-10 Immunization Requirements

Effective July 1, 2009, students are required to have the following immunizations.  Students who are not compliant with these immunizations will need to have a plan for completion in place or they will be denied attendance according to Colorado Revised Statutes 25-4-902.

Required Elementary Student Immunizations

Kindergarten, Grades 1 & 2

  • 5 - DTaP*
  • 4 - Polio**
  • 2 - MMR***
  • 3 - Hepatitis B
  • 2 - Varicella (Or with medical proof of disease history)

3rd – 5th Grade

  • 5 - DTaP*
  • 4 - Polio**
  • 2 - MMR***
  • 3 - Hepatitis B
  • 1 – Varicella

 

* If the fourth dose of DtaP was given on or after the 4th birthday, only four doses are required.
** If the third dose of Polio was given on or after the 4th birthday, only three doses are required.
*** Measles, Mumps and Rubella combination

Required Middle School Student Immunizations

Grades 6, 7 & 8

 

  • 5 – Td/Dt/DTP*
  • 1 – Tdap (new vaccine with Pertussis booster)**
  • 4 - Polio*
  • 2 - MMR***
  • 3 - HB (Hepatitis B)
  • 1 - Varicella (or history of disease)
 

*A child who began immunizations at an older age may be certified with fewer doses.  Check with your school nurse or physician regarding exceptions.
** Tdap is a new vaccine which contains a Pertussis booster (for whooping cough)
***Measles, Mumps and Rubella combination


Required High School Student Immunizations

Grade 9

Grades 10, 11 & 12  

  • 5 - Tetanus/Diphtheria (or 4 doses, if one is
    after the 4th birthday)*
  • 4 – Polio (3, if one dose is after age 4)*
  • 2 – MMR***
  • 3 - HB (Hepatitis B)
  • 1 - Varicella

  • 5 or 4 Tetanus/Diphtheria (as grade 9)
  • 1 – Tdap ** (in addition to grade 9)
  • 4 or 3 – Polio (as grade 9)
  • 2 – MMR***
  • 3 – HB (Hepatitis B)

Contact your family physician for immunizations your child may need.  If you do not have a physician, you can call the Tri-County Health Department at 303-451-0123 to learn where to obtain the required immunizations at low cost. ALWAYS bring a shot record to the immunization clinic.
